Playing subscriptions provide some health insurance cover for injuries sustained while playing or training with MUHC.

There are some gaps and limitations in this cover for certain types of injuries. The club therefore strongly recommends that players should hold private health cover in addition to ensure that minimal costs are incurred in the event of an injury.

Making a Claim

If you wish to make a claim on the Melbourne University Sport Insurance, you will need to complete the forms below:

• University Incident Report S3 Form  
• ACE Insurance - Personal Accident Claim Form 
• Coach's Reference Letter 

The Reference Letter should be completed & signed by your coach. However if your coach was not present at the time your injury occurred, your nominated team captain should complete & sign the letter.

Once completed, return all three forms to:
